Greetings, I hope this message finds you healthy and well. Please read the following information carefully. Failure to follow the instructions below may delay your membership and insurance coverage and cause your volunteer NWJY administrators and National Office staff unnecessary work to process your application.

USJF/NWJY Club Memberships: Club membership applications and fees are due by September 1. Club memberships not processed by the end of September will result in all individual members of that club to be registered as At-Large.

Please send the following forms in PDF format to:

  • 509 Dojo Registration Form (Pages 1 and 2)
  • Delegate Selection form
  • Safe Sport (Annual) https://safesport.org
  • CDC Heads Up or NFHS’s Concussion in Sports program (Valid within 2 years) https://www.cdc.gov/headsup/youthsports/
  • Dojo Membership Roster Authorization form (optional)
  • Insurance Certificate Request form (optional)

Please complete the 509 Registration Form in its entirety. Place “NA” in areas that are not applicable to your club. Please do not skip information assuming it is known or write “same”. Club registration fees are $57. This fee includes USJF and Yudanshakai dues.

Please double check the link.  Dojo’s can pay by either a credit, debit, or PayPal account.

 Individual Memberships: You are strongly encouraged to send individual membership applications directly to the National Office by applying on-line. Be sure applications identify your club and the Northwest Judo Yudanshakai.

Following are the current individual membership fees applicable to the NWJY:

  • Primary Membership – $70
  • Joint Membership, must have primary USJA or USA Judo Membership – $35
  • Primary USJF Life Membership – $35
  • Joint Life Membership, must have primary USJA or USA Judo membership – $0
  • Booster Membership – $15
  • Family Membership (3 members) – $170

 

Important Information on Head Instructor Requirements

The Head Instructor shall be a current member of the USJF, and maintain certifications in Safesport, CDC Heads Up or National Federation of State High School Associations (NFHS) and have a current background check on file. The CDC/NFHS and background requirements are each valid for a 2-year period. Failure to maintain these certifications/background check during the dojo membership year may invalidate your dojo insurance coverage. Failure to have current USJF membership, certifications/background check at the time of dojo registration will cause your application to be rejected by the National Office. Please send certification proof of background check directly to the National Office While it is anticipated the Head Instructor is on the mat the majority of training sessions, these requirements are also highly recommended for all teachers and coaches regardless of certification status.
